if you don't have the means to make your own, such as i, you can get one pre-made.

many people have asked this but i do not know the exact dimension.
1. save as the picture to your desktop
2.open it with microsft picture viewer, editor, w.e
3. click print
4.print wizard should pop up
5.choose full page fax print
6. once print it, cut it out according to the box
