That's true...u nvr know, since this is the 4th game (not counting the NES and SNES games) they may decide to add something to really kick off a new idea.

They obviously know wut they're doing because they've canceled the release twice and they've been working on it for like 6 million yrs. If they don't think adding something this big to the series would help, they won't, if they do they will. It's really that simple.

In my opinion, if there is some "fourth tri-force piece" it would have nothing to do with the twilight realm. Afaik, that is a dark mirror of the normal realm, thus, there would already be 3 dark tri-force pieces in that realm. But meh, I don't know much about the actual story, as they haven't told us much. I'd say a fourth tri-force piece is unlikely. Also, the only reason there is another slot for a fourth tri-force piece in the middle of the tri-force, is because that's the only way they could stack 3 triangles together without it looking stupid.
