Im pretty sure u get it after fighting a mini-boss. I forget which dungeon... Search for a Legend of Zelda guide on Google. That'll help u.

It's from the Frog boss in the Water Temple.

Yup, it's in a chest after you defeat the half way boss in the lakebed temple.

Yeah what they said, it is a mini boss in the water temple, really easy to beat to be honest. I can't remember how you get to it, but i know you have to swim under water in the temple and when you get into the room the circle in the floor where you came out of will seal over and you just need to look up, it's on the ceiling! I've probably confused you more then helped, i'm rubbish at explaining things!
