Plus, I have the suspicion that the medical conditions in the game are fictional, which is a real bummer. I'd prefer a more realistic surgery game.

The majority of the operations deal with real medical conditions. Like the first game though it does branch out in fighting a fictitious virus nearer the end. Actually i think New Blood has more real conditions than Second Opinion had but both are great games anyway.
